Job Title: Case Coordinator
- Job Description: We are committed to helping people with autism, intellectual and developmental disabilities, brain injuries, and neurological conditions become productive members of the community. We are looking for a Case Coordinator to work a hybrid schedule (3 days a week on-site) in Cherry Hill, NJ.
- Location: Cherry Hill, NJ
- Hours: 8:00 AM - 5:00 PM
- Schedule: Monday - Friday
- Type of engagement: W2 contract to hire/ temp to perm
Educational Requirement: Bachelor's degree in Human Services or a related field
Experience: A minimum of two years of related experience in case management and family advocacy is required. Ideally, we are looking for someone with a Nurse Case Manager or Social Work background.
Essential Responsibilities:
- Manage aspects related to assigned caseload, including access to services, family advocacy, and funding, ensuring cases are approached holistically.
- Schedule and coordinate annual plan meetings, ensuring all care team members are included.
- Ensure all documentation for the annual plan is promptly prepared and provided to the appropriate parties.
- Ensure the annual plan accurately captures needed services and that funding authorizations are in place.
- Facilitate eligibility and benefit checks and oversee the process of re-determining and/or restoring eligibility for benefits and entitlements as needed.
- Assist with securing funding authorizations for durable medical equipment and adaptive devices.
- Support the process when individuals transfer services to a new program or service line, ensuring transfer meetings are held with external care team members and all documentation is in place to prevent disruptions in funding.
- Maintain accurate contact information for each individual and their care team in the electronic database (Salesforce). Enter discharges and transfers into Salesforce.
- Oversee the electronic files for assigned caseload.
- Ensure guardianship and Power of Attorney documentation is on file.
- Assist families, individuals served, and residential care teams with resolving medical insurance issues.
- Attend pre-admission and admissions meetings for new admissions to meet external care team members and learn about the service needs of the individual being admitted.
- Coordinate the discharge planning process, preparing the discharge letter and summary.
- Maintain a clear understanding of the funding regulations relevant to the assigned caseload.
